After being decapitated, Art the Clown's headless body ends up at an asylum, where survivor Victoria Heyes, now possessed, gives birth to his head. They kill a nurse and a guard before retreating to an abandoned house, where they enter a dormant state.

Five years later, Art resurfaces during Christmas, brutally murdering a family. Meanwhile, Sienna, released from a hospital, stays with her aunt Jess, struggling with survivor guilt and hallucinations of her deceased friend Brooke. As Sienna tries to reconnect with her brother Jonathan, Art awakens and begins a new killing spree, including a horrific mall attack disguised as Santa Claus.

Sienna learns of Art's return and confides in Jonathan about her fears. After Art kills several students at Jonathan's university, Sienna experiences a nightmare about her family's fate. When Art invades their home, chaos ensues, leading to a violent confrontation. Sienna must fight against Art and his accomplice to save her family while navigating the supernatural horrors unleashed by their presence.
