Lillis began his career as a musician before making his stage debut as Petruchio in The Taming of the Shrew. He made his film debut in Gillian Armstrong's Oscar and Lucinda and appeared in various TV comedies before returning to London in the late 1990s.

In 2006, he portrayed Pope Clement V in Ron Howard's The Da Vinci Code and has since collaborated with contemporary directors like Natalie Erika James and Emma Cooper. Lillis has also worked with artists in projects at Tate Modern and Tate Britain.
