Elisabeth Sparkle, an Academy Award-winning actress, is fired from her job on her fiftieth birthday and has a car accident after seeing her billboard taken down. In the hospital, she receives a flash drive promoting a serum called "The Substance," which claims to create a younger version of the user. After injecting it, a younger version named Sue emerges from her back. The two must switch bodies weekly, with Sue quickly regaining Elisabeth's fame on her old show. As Sue thrives, Elisabeth struggles with feelings of inadequacy and neglects her health, leading to her rapid aging. The relationship between them deteriorates as Sue becomes addicted to the serum. Eventually, Elisabeth decides to terminate the connection, but in a shocking turn of events, she revives Sue, resulting in tragic consequences. The story culminates in a chaotic live broadcast where a monstrous hybrid of the two emerges, leading to a horrifying conclusion that explores themes of fame, identity, and the consequences of desperation.
